We coordinate with their physician or primary health care provider. From there, we build a unique care plan that addresses the specific requirements of the patient.
Often, the care plan will include the transition of the patient from hospital care back to their home environments. Whether the patient only needs basic care after surgery or full-time health supervision by a nurse, our team is more than happy to oblige.
